Donation Request Form Bluestone Bank Donations Our donations program supports organizations local to the communities we serve. Youth activities, educational opportunities, elder services, drug awareness, and fundraisers are just a few examples of the types of town and community events we support. Bluestone Bank reviews requests on a monthly basis. We strongly recommend submitting applications at least 45 days before funds are needed. Organizations requesting support for an event must complete a Donation Request Form. This form will be submitted to the Committee for review. Please attach any supporting documentation to the request along with an IRS tax determination letter indicating 501 (c)(3) status, if applicable.
